Tips for Successful Entries
- Fill out the form neatly (preferably typed)
- Answer all of the questions
- Provide relevant supporting materials
- Organize materials in a binder or folder if necessary
- Programs may be ongoing, but should focus on results from 1/1/12 - 9/30/13
Process and Deadline:
Fill out the form for each entry. Submit four copies of the entry including all supporting materials, enclosing a copy of the entry form with each copy. You may submit as many entries as you wish, in as many categories as you wish. Multiple entries in the same category are also permitted. However, you may not enter the same program in more than one category.
Award winners will announced at the ISHAE Winter Conference in Boston, MA. PSA's with winning entries will be presented with a plaque at the event and will receive complimentary registration to a 2014 ISHAE conference.
